Dog Adoption Application

Thank you for your interest in adopting a dog from Turtle Mountain Animal Rescue. Please read the following closely and answer the following questions. 

 

It may take us up to 1-3 weeks to process your application.

Applicant is under no obligation to adopt a dog, and Turtle Mountain Animal Rescue is under no obligation to provide a dog to applicant. TMAR reviews all applications and will pick the applicant that is the best fit for the dog.  Dogs may be suggested to the applicant based on this application and may differ from dogs requested if we believe that a different dog would be a better match.

All current animals in the home MUST be up to date on all appropriate vaccinations and be spayed or neutered unless there is a medical reason verified by your veterinarian’s office.  

Please call your vet and confirm that your animals are current on their vaccinations PRIOR to completing the application.

Any application that has current pets that are not up to date (excluding medical reasons) will not be considered 

and removed without notice.

 

Once you have adopted you will be asked to arrange appropriate transportation to pick up your dog up at it’s foster home or an agreed upon meeting place within 7 days of adoption.  We do not provide transportation to adopted homes.

 

If you are approved to adopt a TMAR dog but did not get the dog you initially requested, we will send you an e-mail of your pre-approval. Should a dog come up for adoption in the future, you do not need to re-apply, just send us an email to tmaradoptions@gmail.com stating that you are pre-approved and would like to be considered for a specific dog.  Pre-approved applicants will have priority for all newly available dogs listed for adoption. 

 

Please let your references know that we will be calling. Adoptions will not be approved without completed reference checks. TMAR reserves the right to request additional references and complete a background check.

Please know that we are a 100% volunteer organization and it may take a few days to a few weeks to fully process your application,  as we will also verify with your veterinarian and references before we get back to you. Please note that all applicants are required to go through a live interview and home visit before being approved to adopt dogs.

Applications will only be considered after receiving the application deposit for each applicant and not before.

Does anyone in the household that the dog will be residing in have any pet-related allergies?

Current Pets

If you currently have pets (including cats), please provide their information.  Are they Spay/neutered and up to date on core vaccinations?  TMAR requires all pets in the home to be altered and current on vaccinations, unless there is a medical exemption from a veterinarian.  Your application will not be considered if animals in the home are not up to date with core vet care.  You MUST call your vet prior to filling out the application to verify all animals are current.

Veterinary Information

Please indicate your current veterinary information or where you intend to obtain care for your dog.  We will be calling the listed veterinarian to confirm vet care for all current animals in the home.

TMAR Adoption Contract

(In no way does filling out this contract mean that you are approved to adopt a TMAR dog. IF you are approved to adopt a TMAR dog and an adoption is finalized, no further paperwork will be needed.) TMAR will keep record of the adopted dog name and description and this contract. If you wish to have a copy of this contract please send an mail to tmaradoptions@gmail.com. If an adoption is finalized, on the day of the exchange the foster family will provide all relevant known details about the dog including any veterinary care. However, in many circumstances, a TMAR dog will not have known care.
